A wisdom tooth is removed when there is crowding, pain or infection. The procedure is painless under local anaesthetic; healing takes 1-2 weeks. Fly the next day after a simple extraction, 2-3 days after surgery.
Wisdom teeth, or third molars, are the last teeth to develop, usually erupting between the ages of 17 and 25. Most adults have four โ one in each corner of the mouth โ though it is perfectly normal to have fewer, or none at all. By the time they arrive the jaw is often already full, and there may simply not be enough room for them to come through in a useful position.
When space is limited, a wisdom tooth can become impacted โ trapped beneath the gum, angled against the molar in front of it, or only partly erupted. Impaction is the underlying cause of most wisdom-tooth complaints. A partially erupted tooth in particular leaves a flap of gum that traps food and bacteria and is very difficult to clean, leading to repeated inflammation known as pericoronitis.
Removal is not automatic. A wisdom tooth that has erupted fully, sits in the correct position, bites properly against the opposing tooth and can be cleaned easily can be kept and simply monitored at routine check-ups. Extraction is generally recommended when one or more of the following is present:
Recurrent pain or infection around a partially erupted tooth โ often felt as swelling, a bad taste and difficulty opening the mouth.
Pressure on the second molar, which can cause decay or root resorption in the otherwise healthy tooth in front.
Decay in the wisdom tooth itself that cannot be cleaned or restored reliably because of its position.
Cysts or other lesions developing around an impacted tooth.
Localised gum disease that does not respond to cleaning.
The decision is always based on imaging. A panoramic X-ray shows the position and angle of the roots, and for lower wisdom teeth lying close to the inferior alveolar nerve, a 3D cone-beam CT scan maps that relationship precisely and reduces surgical risk.
The entire procedure is carried out under local anaesthetic, so you will not feel pain during the extraction โ only some pressure. For very anxious patients, or when several teeth are removed together, additional sedation can be discussed in advance.
A fully erupted wisdom tooth can often be removed in a few minutes with a simple extraction: the tooth is gently loosened and lifted out. An impacted tooth requires a small surgical procedure โ a tiny incision in the gum, sometimes removing a little bone, and occasionally dividing the tooth into sections so it can be eased out without stress on the surrounding bone. Dissolvable stitches are usually placed afterwards.
A gauze pad is placed over the socket and you bite down to control bleeding while a protective blood clot forms. This clot is essential โ it is the foundation of healing, which is why the aftercare instructions below focus on protecting it.
First 24 hours. Mild bleeding, swelling and discomfort are normal. Bite gently on the gauze, apply a cold compress to the cheek for 15-20 minutes at a time, rest with your head slightly raised, and take the prescribed painkillers before the anaesthetic fully wears off.
Days 2-3. Swelling and any jaw stiffness usually peak around this time, then begin to ease. Begin gentle warm salt-water rinses after meals to keep the area clean, and keep to soft, lukewarm foods such as soup, yoghurt, eggs and mashed vegetables.
Days 4-7. Most people feel substantially better and return to normal activities. Any dissolvable stitches typically loosen or dissolve within one to two weeks.
Aftercare โ the essentials:
Do keep the area clean with warm salt-water rinses from day two onward.
Do take any prescribed antibiotics and painkillers exactly as directed.
Don't smoke or drink through a straw โ the suction can dislodge the clot and cause a painful dry socket.
Don't rinse vigorously, spit forcefully or do heavy exercise for the first few days.
Serious complications are uncommon, and most can be avoided with careful technique and good aftercare. The most frequent is dry socket (alveolar osteitis), where the protective clot is lost early, exposing the bone and causing throbbing pain a few days after the extraction; it is treated easily and resolves with a medicated dressing. Temporary numbness of the lip or tongue can occur when a lower tooth lies very close to the nerve โ which is precisely why we use 3D imaging beforehand to assess and reduce that risk. We further lower the chance of problems through gentle, planned surgery, clear written aftercare and prompt remote follow-up.
If you are travelling for treatment, timing matters. After a simple extraction you can usually fly the next day. After surgical removal of an impacted tooth, we generally advise waiting two to three days so any swelling settles and the clot is stable.
